A Case of Paget¡¯s Disease Misdiagnosed As Ankylosing Spondylitis

Abstract
We report here on a 43-year-old female patients with Paget¡¯s disease involving the whole spine, pelvis and femur. She admitted because of progressive lower back pain with morning stiffness during 1 year and these improved by the exercise. Physical examination revealed positive Shober test with localized tenderness on hip joint & lower back. Paget¡¯s disease is a localized disorder of bone remodeling. Increased osteoclasts and their activity initiate the process at affected skeletal sites, and the increase in bone resorption is followed by increased new bone formation, altering bone architecture. The signs and symptoms of Paget¡¯s are varied, depending on the location of the involved sites and the degree of increased bone turnover. Patients with Paget¡¯s disease have increased urinary excreation of deoxypyridinoline and elevated serum alkaline phosphatase. In this case, initial serum alkaline phosphatase and deoxypyridinoline has been increased but both of them were much decreased after treatment with pamidronate 60 mg for 3 days.
